JOB SUMMARY

The primary purpose of this job position is to coordinate and facilitate the patient case load from admission through discharge. Care Managers are also responsible for initiating and maintaining contact with 3rd party referrals, families of patients, and making appropriate aftercare referrals and placement.

QUALIFICATIONS

A Bachelor’s degree or Master’s degree is preferred; one-year experience as an Admissions role or Recovery Coach at an addiction treatment facility with a CADC/CADC Intern. Must understand or be willing to learn the disease of alcoholism and chemical dependency and its treatment; knowledge of managed care procedures, DSM-V diagnosis and ASAM Patient Placement Criteria; computer skills associated with data entry; knowledge of billing procedures. Must possess knowledge of crisis counseling and intervention skills. Excellent customer service skills with ability to effectively communicate and interact with internal and external customers. Must participate in initiating, performing, and maintaining activities to improve organizational performance. Must attend meetings and/or training sessions outside of working hours, as designated. Must be willing to accept assignments between appropriate departments and/or temporary assignments to other facilities.

Work schedule will include rotating days,evenings, weekends and holidays
